Our VNA of Pottstown Medical Social Work team provides intervention when social, emotional or environmental issues are interfering with a patient’s health care treatment or recovery. Social workers provide counseling, long-term planning, education and referrals for benefits and entitlements, community resource connection, placement and housing referral advocacy, discharge planning and brief care-focused family counseling.
