Hospice Care is the specialized comfort care provided to the terminally ill. It is a holistic, patient-centered approach which includes physical, spiritual, and psychosocial care and support for both the patient and family.
Care is provided for a wide variety of disease processes in many different community settings such as homes, skilled care nursing facilities and hospitals. The Hospice Team is made up of caring and compassionate nurses, social workers, a spiritual counselor, home health caregivers, and dedicated volunteers, and includes the assistance of other VNA of Pottstown services when necessary.
The VNA of Pottstown also provides bereavement support for 13 months after your loved one has passed away. We understand that grieving is a process and you don’t have to go through it alone. Families have the option to speak one on one with our spiritual counselor, attend our support group meetings, or participate in special events in memory of loved ones.
